1) Fill out the PTA Membership Form using the link to the right. 

 

2) Return the completed PTA Membership Form and Payment to the PTA drop box or in your child's red folder.

 

3) Make checks payable to Star Lake PTA.

 

4) The PTA will notify you that your application has been processed
